It’s two in the morning again. The day was fine and tonight your body has decided otherwise, and tomorrow you’ll do it all on four hours.

Not sleeping is reason enough on its own. So is being worn down by work, without anything dramatic having happened to explain it.

Anna Salino-Hugg is a Licensed Clinical Social Worker at Allora Rise Counseling and Therapy, seeing adults 18 and up at the practice’s Wrightsville Beach office. Her clients are most often young professionals and students finishing college and starting careers, people dealing with relationship and interpersonal conflict, and clients who can’t sleep.

She works most often with three things.

Insomnia

You’re awake again at two, and tomorrow you'll be running on fumes. Sleep that won’t come is treatable, and the work is CBT for insomnia, specific and time-limited rather than open-ended, and clients seek her out for it by name.

Chronic stress and work-life balance

If you work in one of the helping professions, you’re in good company here. Much of her caseload is nurses, teachers, and public safety staff, and what they want is the stress brought down and a work-life balance that actually holds. That means burnout, chronic stress, and the point where a job stops leaving room for the rest of a life.

Anxiety, including panic

Panic attacks, social anxiety, health anxiety, or the kind that turned up when you got the promotion. Anna treats panic directly rather than working around it, and she also sees depression and mood concerns where they coincide.

Beyond those three, Anna also works with perfectionism, trauma and post-traumatic stress disorder (PTSD), grief and traumatic loss, ambiguous loss, family conflict, and life transitions. She also incorporates faith into the work for clients who want it there.

Anna spent nearly a decade working with veterans and their families across treatment settings in the VA Healthcare System, and she has deep experience with PTSD and with serious mental illness populations.

Sessions are serious when they need to be and light when that’s what helps. Once rapport is built she’s direct, roughly matching how direct you are.

She’s trained in Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), motivational interviewing, Gottman and emotionally focused couples work, Prolonged Exposure for PTSD, IPT, and CBT-Insomnia (CBT-I). In practice she uses CBT, ACT, IPT, motivational interviewing, and solution-focused approaches, chosen around the client rather than the diagnosis. She reaches for IPT for the role transitions that come with graduation and early career. On grief she often uses IPT, where the work usually runs twelve to sixteen sessions, and she uses an analogy of an acute wound healing to a scar to describe the grief work process, as your relationship to the loss changes over time.

She’s from the Midwest, and where it’s useful she has been open about starting over as an adult later than planned, and about infertility and grief. Alongside her caseload she provides clinical supervision for LCSWAs in North Carolina and LMSWs in Iowa, and case consultation to other therapists and health care providers.

Your first session

Anna welcomes you and checks how you’re feeling about being there at all, which isn’t a formality. Then the housekeeping: how the practice works and what therapy involves. She will tell you in advance that she’s going to ask a lot of questions, and you’ll go through the intake materials you completed beforehand together. At the end you discuss next steps, set an initial treatment goal, and schedule. If she isn’t the right person, she will help you find who is.
